Sage's Assessment and Support program helps older adults and their family members to connect with resources, services and supports. The goal of the program is to ensure that the needs of the seniors and families that we work with are being met in all areas of life.
The social work staff in the program offer assistance or resource information with many issues such as:
- health
- grief
- finances
- relationship issues
- mental health/emotional well-being
- elder abuse
- decision making options
- legal matters
- safety
- social/recreational
- and more!
In addition, the social work staff help seniors with filling out the following forms: Canada Pension Plan (CPP), Old Age Security, Guaranteed Income Supplement (GIS), Alberta Seniors Benefits, Special Needs Assistance, and Housing Applications.
*Before meeting with a social worker at Sage to fill out forms, please note that these forms require information that you may not carry around with you, such as your Social Insurance Number (SIN), banking information and specific dates.

You can also call 780-423-5510, extension 323 or 587-773-1764 to leave a voicemail, and one of our social workers will return your call within 2 business days. Home visits can also be arranged.
