SAFE HOUSE COORDINATOR II, SENIORS' SAFE HOUSE
The Safe House Coordinator II will work closely will work as part of the Seniors’ Safe House team and liaise with the transition suites housing provider to facilitate services to clients. The Coordinator will coordinate accommodation, services and case management for clients including intake, assessments and safe discharge planning. This role will also help to develop program polices, procedures and processes.
QUALIFICATIONS, SKILLS and EXPERIENCE
• Registered Social Worker is preferred
• Must provide an enhance criminal record check from the Edmonton Police Service
• Experience working with older adults
• Familiarity with social and psychological issues related to elder abuse and neglect
• Excellent time management skills
• Experience in Social Work case management including assessing client needs and related risks
• Class 5 Drivers License and Vehicle are required
• Self-motived and able to work both independently and as part of a team
This position is a temporary, 6 month, part-time (0.8 FTE) term opportunity with the possibility of becoming permanent. The starting salary range is between $23.32 – 27.45/hr. Wage considerations are based on assessment of the successful candidate’s education, skills, previous experience and other factors. Sage offers an outstanding work life balance with a 35-hour work week, generous paid sick time, and paid time off between Dec 25 – Jan 1st. Sage also observes 13 statutory holidays in the calendar year. LINK WORKER, Social Prescribing Program (Temporary)
The Link Worker will work with Individuals referred by medical professionals to the Social Prescribing to provide support through connecting older adults to daily living and social support in the community. This could include assistance in accessing food resources, transportation, physical activities, volunteer opportunities or assistance in filling out forms. The Link worker will provide direct support by providing information or attending appointments with the client. The goal of this position is to assist older adults to age safely in place.
QUALIFICATIONS, SKILLS & EXPERIENCE
• Knowledge of issues impacting older adults and the community resources available
• Proficient problem-solving skills and the ability to make informed decisions
• Computer literacy including Microsoft Office
• Proficient oral and written communication skills and organizational skills
• Police Information Check including Vulnerable Sector is required
• Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
• Experience working with older adults and volunteers
• Previous experience as a community worker is an asset
• Experience working with diverse cultures
The position is a temporary, full-time position to April 30, 2025 with some flexibility for 0.8 FTE to full-time hours depending on preference. The starting rate is $22.32/hr. SOCIAL WORK CASE COORDINATOR, Social Prescribing Program (Temporary)
The Social Work Case Coordinator will address the social and emotional needs of older adults through assessment, support, and community-based case management. The Coordinator will work with individuals in a supportive, holistic way to address barriers to access in areas including finances, housing, transport, low confidence and social isolation. Referrals to this project will be received from health professionals. The emphasis of the program is to provide social supports to assist the older adult to remain healthy in a holistic way in the community.
QUALIFICATIONS, SKILLS & EXPERIENCE REQUIRED
• Registered Social Worker with ACSW
• Relevant degree or diploma; Bachelor of Social Work is an asset
• Experience in community-based case management
• Strong assessment, interpersonal, problem-solving and decision-making skills required
• Self-motivated; able to work independently and as part of a team
• Computer literacy including Microsoft Office
• Proficient communication and collaboration skills
• Police Information Check including Vulnerable Sector is required
• Vehicle required; valid class 5 driver’s license and $2 million auto liability insurance
