Our mission at Sage is to inspire and support seniors to be the best they can.
To accomplish this, our interdisciplinary team employs a compassionate, holistic and person-centered approach to work with seniors in our community.
Social workers are an integral part of Sage's service delivery team. Their scope of practice entails amplifying the voice of seniors by assessing their needs, providing resources and system navigation support, advocacy, case management, and assisting in completing paperwork required to access eligible benefits.
